Below is a form to fill out by a truck driver who has committed a traffic violation.

Within 2-3 weeks from getting a ticket you should receive a written citation from court regarding the bail amount and other possible court fees. There should also be information on traffic school options. Successful completion of a traffic school in most cases will result in the dismissal of the citation.
